STARTUP & COMMISSIONING REQUEST

Please allow 2 weeks for scheduling after receipt of the completed form

1. PROJECT & SITE INFORMATION

If multiple stations require commissioning, submit a separate request for each station.

2. REQUIREMENTS — READ BEFORE SCHEDULING

  • We recommend the site electrician and 2 laborers be on-site and available to assist throughout the duration of the startup and commissioning.
  • A water source is required to test the station during startup. Water must be provided by a 2" or larger hose. A residential garden hose is not recommended.
  • Wet well and piping must be clean and free of debris.
  • Electrical cables must be pulled to the station control panel.
  • Station must not be exposed to sewage prior to commissioning. Contractor will be held responsible to handle any equipment which has been in contact with sewage.
  • Station training should take place immediately following completion of commissioning.
  • Contractor will be responsible for all additional costs required for return visits due to incomplete construction, unavailable or absent crew members, repeated testing, and owner training.

3. SITE READINESS QUESTIONS

1. Will a lifting device to remove pumps be available during commissioning?
2. Has permanent power been provided to the control panel?
3. Has electrical conduit been sealed?
